MENDOTA – Parents that have not already registered their student for the 2019-20 school year at Mendota High School should do so online at www.mendotahs.org. If you do not have access to the internet please contact the school at (815) 539-7446.
In addition, all required immunizations are to be completed before the start of school. Any student lacking these immunizations will not be allowed to continue the school year until the school has received proof of immunization.
All incoming freshmen and new students from out-of-state are required to have a physical before attending classes at Mendota High School. Local doctors have the necessary forms or they are online at www.mendotahs.org. Completed forms are to be turned in during Freshman Orientation on Wednesday, Aug. 14 or to the Principal's Office prior to the first day of classes on Friday, Aug. 16. Per Illinois School Code, parents are required to fill out and sign the health history section of the “Certificate of Child Health Examination.”
If your student requires medical assistance, please complete the medical form available on the MHS website. This includes but is not limited to asthmatic, diabetes or anaphylactic conditions.
All students participating in a fall sport should get their physical as soon as possible. Sport physicals need to be renewed each year per the Illinois High School Association Rules.
Any student planning to participate in a fall sport must have a physical examination form turned in to the nurse or principal’s office (not the coach) prior to the start of practice. No one is allowed to practice or participate without a physical examination on file with the school.
Seniors only: A second dose of the Meningococcal Vaccine is required at the start of senior year or after turning 16 years of age. One dose required if received after age 16. No proof of immunity is allowed.
